- Try Control Center → Code Scanner for difficult angles
- Increase distance slightly — close focus can blur small codes
- Disable excessive lens flare: tilt phone or reduce glossy glare
- Update iOS — older versions had weaker auto-detect in low light
When the code is the problem

- Quiet zone clipped in artwork
- Broken redirect — test URL manually in Safari
